Individual membership includes:
House calls, telemedicine, biannual basic labs, point-of-care testing, basic procedures including supplies, OMT and cupping as indicated
Membership fee is not insurance
Our annual membership is not an insurance product, the fee covers ongoing primary care and the services described herein, and you are responsible for any additional services pertaining to your/families health not included in membership
In an emergency call 911 or go to the nearest emergency department
The annual membership is not suitable for any emergency

Membership fees are billed annually. Refunds when approved, are prorated by quarter under specific conditions

Geographic limitations
Services available to families residing in Fishers, Carmel, Westfield, Noblesville or within a 5 mile bordering radius; outside this area additional fees may apply
